In accordance with the law, in order for your child to attend school, all of the following papers must be completed and on school premises for Department of Children and Families to review. Your child will not be able to attend school if we do not have this paper work on file. We appreciate your help in this matter.

Blue Certificate of Immunization (provided by doctor)
Yellow Student Health Examination Florida Department of Children and Families form (provided by doctor)
Know Your Child Care Center brochure (signed on back)
Disciplinary Procedures and Expected Conduct form (signed)
